By agreeing to the items listed below when submitting the new registration and/or registration renewal form the applicant hereby certifies that he/she:

1. is licensed and in good standing to practice law in the State of North Carolina; and,
2. currently devotes a minimum of 25% of his/her practice to representation of creditors; and,
3. pledges to support the stated purposes and policies of the Association.
